This form will be returned to you by fax with an RMA number, Products shipped to Prime Computer Systems without an RMA number will be refused. The RMA number must be clearly marked on the outside of the package. Damage or loss of goods during shipment is the sole responsibility of the customer. Insufficient packaging may result in loss of warranty. RMA number is valid for 30 days. Products returned for credit are subject to a 15% restocking fee.
